• A referral letter from your GP, other specialist / Emergency doctor

                (NB: a referral is not required for hand therapy)

• All x-rays / MRI / CT scans and reports or other relevant imaging of the affected region
• Your medicare card / DVA card
• Your private health insurance card
• WorkCover claim number (if applicable)
• Previous Operation reports (if applicable)
• Any other relevant information about your condition
• Payment method for the consultation (see billing)

Dr Graham , Dr Loch-Wilkinson, Dr Williams and The Palms Hand Therapy all consult at the Mudgeeraba Hand Clinic - Suite 4/75 Railway St Mudgeeraba QLD 4213.
